Travel insurance for USA from UK is not legally required on an ESTA, but US hospital bills are high and a GHIC does not work there. Here is how to choose.
You do not legally need travel insurance to visit the USA from the UK on an ESTA, but the Foreign, Commonwealth and Development Office warns that medical treatment there is expensive and that there are no special arrangements for British visitors. Your GHIC does not work in the United States. That leaves two routes: a UK travel policy with a high medical limit, or a US visitor medical plan, and which one fits depends on your age, your health and what else you want covered.
Key facts at a glance
- The FCDO's USA health advice says medical treatment is expensive and there are no special arrangements for British visitors.
- GOV.UK's foreign travel insurance guidance gives £150,000+ as an example cost for a stomach bug treated in a US hospital, with possible evacuation or repatriation.
- The US is not on GOV.UK's list of countries with a UK reciprocal healthcare agreement, and a GHIC is not a substitute for travel insurance.
- CBP's Visa Waiver Program allows stays of up to 90 days for tourism or business with an approved ESTA.
- GOV.UK warns that failing to declare existing conditions or pending treatment may invalidate a UK travel insurance policy.
- The FCDO notes that an ESTA without the selfie photograph now required by CBP may need to be replaced with a new application.
Do I need travel insurance for the USA from the UK?
There is no legal requirement: US law does not require ESTA visitors to carry health cover. The practical answer is different. As the FCDO puts it, there are no special arrangements for British visitors, so the bill for any treatment is yours or your insurer's. GOV.UK's guidance puts it bluntly, listing £150,000+ as a possible cost for a stomach infection treated in a US hospital with evacuation or repatriation.
A useful US reference point is AHRQ's Healthcare Cost and Utilization Project, which puts US hospitals' own cost for a heart attack stay at roughly $28,000 in 2022, before physician fees and before any markup on the bill. Our guide on whether you need travel insurance for an ESTA trip covers the entry side in more depth.
How much medical cover do you need for the USA?
Choose a limit that could absorb a serious admission plus a medical flight home. On US visitor plans, WorldTrips' Atlas America offers overall maximums from $50,000 to $2,000,000 for travelers under 65, and IMG's Patriot America Plus offers $50,000 to $1,000,000 through age 69. Older travelers face lower caps: both plans limit travelers aged 70 to 79 to $50,000 or $100,000.
On a UK policy, find the medical expenses figure on the policy schedule and check that repatriation is included. You can compare costs for a US emergency with our medical cost estimator before deciding.
UK holiday policy or US visitor medical plan: what is the difference?
A UK single trip or annual policy is usually a bundle: medical expenses and repatriation, plus trip cancellation, baggage, personal liability and sometimes gadget cover, priced in pounds and sold under UK rules. It is often the natural choice for a two week holiday that you would lose money on if you had to cancel.
A US visitor medical plan is built around US healthcare. It typically has a deductible in dollars, a US PPO network (UnitedHealthcare on IMG's Patriot America plans), set copays for urgent care and extra charges for using the emergency room for an illness without admission. It is primarily medical cover, so it may not replace cancellation or baggage cover. Visitor plans are often considered for longer stays, for a stay split between the US and other countries, or when you want access to a US network. Our comparison of visitor insurance and travel insurance goes through the trade offs.
What is the best or lowest priced travel insurance for the USA from the UK?
There is no single best policy; the right one covers your trip at a price you accept. On US visitor plans, premiums depend on age, trip length, policy maximum and deductible, and a higher deductible lowers the premium but raises what you pay first. A UK policy's excess works in a similar way. The lowest priced option is only good value if its medical limit, repatriation cover and treatment of your health conditions match your needs, so compare those three items before price.
What about pre-existing medical conditions?
This is where the two routes differ most. A UK insurer will ask you to declare your conditions and may cover them for an extra premium; GOV.UK stresses that you must declare existing conditions or pending treatment and tests, or risk the policy being invalidated.
US visitor plans exclude pre-existing conditions and offer only an acute onset benefit for sudden, unexpected flare ups, within age limits and sometimes sublimits. Trawick's Safe Travels USA Comprehensive, for example, caps acute onset claims for cardiac conditions or stroke at $25,000 through age 69. If you have a significant ongoing condition, a UK policy that screens and accepts it may protect you better. Our guide to travel medical insurance with a heart condition shows how these limits work in practice.
Which US visitor plans can UK residents buy?
According to Ombrela's plan catalog, IMG's Patriot America Plus and Patriot Platinum and Trawick's Safe Travels USA plans do not exclude UK residents. WorldTrips plans such as Atlas America are listed as unavailable to people physically located in the United Kingdom at the time of purchase, and WorldTrips caps UK residents and citizens at 119 days of coverage. For an ESTA trip of up to 90 days bought from home, that points most UK buyers toward the IMG and Trawick options. Confirm eligibility for your own residence and location on the quote page before you buy.
What should you know about US pharmacies and urgent care?
Pack enough of every prescription medicine for the whole trip plus several days, in your hand luggage, as the CDC recommends. The FCDO notes that some prescription drugs cannot be taken into the US and points travelers to the FDA's guidance, so check before you fly. Replacing a lost UK prescription in the US may mean seeing a US clinician first.
For minor illness or injury, an urgent care clinic is quicker and far less costly than the emergency room; for severe chest pain, severe breathlessness or heavy bleeding, call 911. Our guide to urgent care that takes travel insurance explains what to say at the front desk.

How to get covered
If a US visitor medical plan suits your trip, enter your dates, ages and US state on the visitor insurance quote page and compare plans that accept UK residents. Check the policy maximum, deductible, network and acute onset limits for each traveler's age before you buy.
Frequently Asked Questions
Does my GHIC or EHIC work in the USA?
No. The UK Global Health Insurance Card and the UK EHIC give access to state healthcare in the EU and a limited list of other countries, and the United States is not among them. GOV.UK also says these cards are not alternatives to travel insurance, since they do not cover private treatment or repatriation. For a US trip, you need a travel policy or a visitor medical plan.
Is annual multi trip insurance enough for a US holiday?
It can be, if the policy includes worldwide cover with the USA, its medical limit is high enough, and each trip fits within the maximum trip length the policy allows. Check that the area you chose on the policy includes the USA, declare any health conditions, and confirm that your planned stay fits within the policy's per trip limit before you book.
Can UK residents buy US visitor insurance after arriving in America?
Some US visitor plans can be purchased after arrival, and some WorldTrips plans can only be bought once you are no longer physically in the UK. Buying after arrival leaves you uninsured on the flight and your first days, and any illness that starts before the effective date is excluded. The clearest approach is to buy before departure from a plan that accepts UK residents, with coverage starting on your travel date.
Does a US visitor plan cover trip cancellation?
Generally no. US visitor medical plans focus on medical expenses, emergency evacuation and repatriation, with some adding smaller extras such as lost checked luggage on certain plans, according to Ombrela's plan catalog. They are not designed to refund prepaid flights and hotels if you cancel before leaving. If cancellation cover matters to you, a UK travel policy, or a separate cancellation policy, fills that gap.
A US trip from the UK deserves a medical limit sized for US hospital bills. Check your ESTA, declare your conditions, and compare visitor insurance plans on Ombrela if a US medical plan suits the way you travel.
