Japan does not require tourists to hold travel insurance to enter as of October 2026, although the government has said it is studying such a rule. Visitors are outside the national health insurance system, and the US Embassy says providers expect payment in full at the time of treatment. Travel medical insurance with evacuation cover is the product that fits. It is not trip cancellation cover.

Do you need travel insurance for Japan?
Not to get in. No published entry rule in force in October 2026 asks short term visitors to show a policy at the airport, and US citizens can visit for up to 90 days without a visa.
That may change. The Japanese government has said it is studying whether to require visitors to carry insurance, driven by hospital bills left unpaid by tourists. No start date had been published at the time of writing. Japan has also announced a future electronic travel authorisation system for visa exempt travelers. Check official guidance again before you fly.
Two things are already in place:
- The Japan National Tourism Organization tells visitors to sign up for travel insurance with adequate cover before departure.
- The US Embassy reports that Japan has introduced enhanced immigration screening for visitors with unpaid medical fees who attempt to enter again. An unpaid bill can follow you.

How does healthcare in Japan work for visitors, and what does it cost?
The standard of care is high. The friction for visitors is language, payment and how the system is organised.
- You pay in full. The US Embassy states that caregivers require payment in full at the time of treatment, or concrete proof of ability to pay, from anyone who is not a member of the national health insurance plan. You must settle the bill before leaving hospital and then seek reimbursement from your own insurer.
- Cash still matters. The Japan National Tourism Organization's medical guide notes that credit cards are accepted only at major hospitals and that clinics generally take cash only.
- English is limited. The embassy describes English speaking physicians and facilities as expensive and not widespread.
- Clinics first. For ordinary illness you visit a clinic or general hospital in regular hours. Emergency hospitals are for urgent cases.
The tourism organization runs an online search for medical institutions that accept foreign patients, filtered by area, language and department. Institutions certified under the JMIP scheme have been assessed by a third party on how they receive foreign patients.

What are the main health and safety risks in Japan?
Japan carries a Level 1 advisory from the US State Department, the lowest of four levels. The risks worth planning for are mostly environmental and medical.
Earthquakes and tsunami. Japan is one of the most seismically active countries in the world. Learn the evacuation route at your hotel and follow official alerts on your phone.
Summer heat. July and August are hot and humid in most of the main islands, and heat illness sends many people to hospital each summer. Plan indoor breaks and drink water regularly.
Medication rules. The US Embassy warns that many common medications and over the counter drugs in the United States are illegal in Japan, and that carrying them risks arrest even with a valid US prescription. Quantities above the approved limit need an import certificate called a Yunyu Kakunin-sho, obtained before travel. US prescriptions are not honored in Japanese pharmacies, so bring enough of any permitted medicine for the whole trip.
Mountain and snow sports. Skiing in Hokkaido or Nagano, hiking in the Japan Alps and climbing Mount Fuji all carry injury and altitude risk. Check whether your plan covers these as standard. Our guide to adventure sports cover explains optional riders.
Vaccines. CDC asks all international travelers to be fully vaccinated against measles, and suggests Japanese encephalitis vaccine for long stays or frequent rural travel.
When is typhoon season in Japan?
Typhoons can form in the western Pacific in any month, but they affect Japan mainly from about July to October, with August and September the busiest. Okinawa and the other southern islands are hit earliest and most often. Kyushu, Shikoku and the Pacific coast of Honshu follow.
A typhoon usually means a day or two of suspended trains, cancelled flights and closed attractions. Rail operators often announce planned suspensions ahead of time, so follow official weather warnings and operator notices.
Know what your policy does in a storm. Travel medical insurance pays for treatment if you are injured. It is not built to refund flights or hotels. Some plans Ombrela lists include modest trip delay or trip interruption benefits, but they are limited and have conditions. Trip cancellation insurance is a different product.
What should a travel medical plan include for Japan?
- A medical maximum sized for a hospital stay you pay in full. Because you settle the whole bill, pick a maximum that would cover surgery and several nights of inpatient care.
- Emergency medical evacuation. Japanese national insurance does not pay for it even for members, the US Embassy notes. Most international plans in Ombrela's catalog list evacuation benefits up to 1,000,000 US dollars. Read how evacuation cover works.
- An assistance line that can speak to the hospital and, where possible, arrange a guarantee of payment.
- Acute onset of preexisting conditions, if you have a known condition. Benefits and age limits differ sharply between plans.
- Winter sports, if you plan to ski or snowboard. Some plans offer an optional adventure sports rider.

How do you use your insurance if you get sick in Japan?
- In an emergency call 119 for an ambulance. Police are on 110. The tourism organization's guide notes that you cannot choose which hospital an ambulance takes you to.
- Need help in English? The Japan Visitor Hotline run by the Japan National Tourism Organization answers 24 hours a day on 050-3816-2787, in English, Chinese, Korean and Japanese.
- Call your insurer's assistance number early. Ask whether it can arrange cashless treatment at a hospital near you. The tourism organization's guide says that if your travel insurance includes a cash free payment service you do not pay at the hospital, and otherwise you may be charged the full amount.
- Ask reception for an estimate before treatment, as the same guide suggests.
- Keep every document. You need the itemised bill, the receipt and a medical report or diagnosis to claim. Ask for English versions if the hospital can provide them.
- Pay before you leave. Unpaid fees can lead to extra screening the next time you try to enter Japan.

Japan is a member of the US Visa Waiver Program, so most Japanese citizens visiting the United States for up to 90 days of tourism or business travel apply online for ESTA approval instead of a visa. Longer visits need a B-2 visitor visa. Neither route gives access to US public health programs, and Japan's national health insurance reimburses overseas care only up to what the same treatment would have cost in Japan, which can be far below a US bill.
