Do you need travel insurance to visit the Dominican Republic?
Not to get in. The document every traveler must have is the electronic entry and exit form, usually called the e-ticket. It is free on the Dominican migration authority's website and produces a QR code. The Government of Canada says you must complete it before boarding your flight, and a new form is needed for each trip. Be careful with lookalike websites that charge a fee.
Insurance is a recommendation, and a firm one. The State Department advises travel insurance and reminds US citizens that Medicare and Medicaid do not apply abroad. Medicare.gov confirms that Medicare usually does not cover care outside the United States. The UK Foreign Office goes further and warns that visitors who cannot pay a hospital bill may be prevented from leaving the country until the debt is settled.

How good is healthcare in the Dominican Republic, and what does it cost?
There are two systems. The UK Foreign Office says public facilities are generally limited, while private hospitals offer a better standard at higher cost. The Government of Canada says good care is only available in major cities and that quality varies greatly across the country.
Resort areas such as Punta Cana, La Romana and Puerto Plata are served by private clinics and hospitals that are used to treating tourists. Canada's advice includes two cautions worth knowing. Private facilities may overcharge, so ask administrators about fees before treatment where you can. And resort doctors on site may use aggressive sales tactics.
Canada says facilities may require payment up front. The US Embassy in Santo Domingo keeps a list of medical providers on its medical assistance page. What are the main health and safety risks in the Dominican Republic?
The State Department's Level 2 advisory from June 2025 cites violent crime, while noting a stronger police presence in tourist areas. Check the current level before travel. For health claims, these are the recurring causes:
- Road accidents. The UK Foreign Office says the country has a high death rate from road traffic accidents and that motorbike taxis, known as motoconchos, are often driven badly and may not offer a helmet.
- Stomach illness. The UK Foreign Office advises against drinking tap water. Stick to sealed bottled water and follow CDC food guidance.
- Mosquito borne illness. CDC lists dengue and Zika. It also lists malaria risk in certain provinces, so check CDC's Dominican Republic page against your itinerary and ask a travel clinic whether preventive medicine applies.
- Water. The UK Foreign Office notes lifeguards and rescue equipment may be absent at pools and beaches.
- Rabies. CDC advises washing any animal bite or scratch immediately and seeking care promptly.
People traveling for surgery or dental work should know that travel medical plans generally exclude planned treatment and its complications.
When is hurricane season in the Dominican Republic?
June 1 to November 30. Storms are most frequent from August to October. The Government of Canada tells travelers to be ready to change plans at short notice during the season and to follow local forecasts.
Heavy rain causes flooding and landslides even without a named storm, and airports on the north and east coasts can close with little warning.
A travel medical plan is for the medical consequences: treatment if you are injured or ill, and evacuation when doctors say it is necessary. It does not refund flights or resort nights lost to a storm. For that you need trip cancellation cover, bought before a storm is named. What should a travel medical plan include for the Dominican Republic?
- A medical maximum suited to private hospital billing, since private care is where visitors are normally treated.
- Emergency medical evacuation. Canada and CDC both recommend it, because serious cases outside the main cities may need transfer to Santo Domingo or out of the country.
- A 24 hour assistance line that can review a hospital estimate and arrange a payment guarantee. This is useful given the official warnings about overcharging.
- Activity cover for what you actually plan: buggies, zip lines, catamaran trips, diving. Check the exclusions, especially for motorbikes and all terrain vehicles.
- Acute onset of pre existing conditions if you have a chronic condition.

How do you use your insurance if you get sick there?
Call 911 in an emergency. The US Embassy says 911 works in greater Santo Domingo and other major metropolitan areas, and coverage is thinner in rural areas. In tourist zones you can also call the tourist police, CESTUR, on 1 809 200 3500.
- Contact your plan's assistance number before agreeing to tests or admission if the situation allows.
- Ask for a written estimate. Let the assistance team speak to the hospital's billing office.
- Pay any deposit by card, not cash, and keep every receipt.
- Get the medical report and itemized invoice before discharge, in English if available.
- Do not hand over your passport as security for a bill. Call the assistance team and your embassy if this is requested.
