Travel Insurance From China to USA: Parents and Visitors
⚠
Travel insurance from China to USA explained: plan length for B1/B2 trips, paying from the US, acute onset cover for parents, and language help.
Travel insurance from China to USA is a visitor medical plan that pays for new illnesses and injuries during a stay in the United States, and it can be bought online by the traveler or by family already in the US. For parents and relatives on B1/B2 visas, the usual choice is a comprehensive plan sized to the full stay, with an acute onset benefit that still applies at their age. Residents and citizens of China are eligible for the visitor plans compared on Ombrela.
Key facts at a glance
- The United States does not legally require B-2 visitors to hold health insurance. It is optional, and the visitor is personally responsible for any medical bill.
- According to US Customs and Border Protection, travelers on a People's Republic of China passport with a ten year B1, B2 or B1/B2 visa must hold a valid EVUS enrollment to travel. EVUS is not insurance.
- An EVUS enrollment generally lasts two years, or until the passport or visa expires, whichever comes first, according to CBP.
- CBP records the permitted stay on the I-94 at entry, commonly up to six months for B-2 visitors. That date, not the visa validity, is the outer limit of the trip you insure.
- Under WorldTrips' published limits, Atlas America caps the policy maximum at $50,000 or $100,000 for ages 65 to 79 and offers acute onset cover only to travelers under 80.
- The plan administrators we compare advertise multilingual emergency assistance around the clock. None of them promise a Mandarin speaker on every call, so plan for interpretation.
Do Chinese visitors need travel insurance for a US visa?
No. A B1/B2 visa application does not ask for a policy, and the EVUS enrollment that ten year visa holders complete before travel has nothing to do with medical cover. CBP began charging a fee for EVUS enrollment in September 2025, set at $30 at launch and adjusted for inflation each year, so check the official EVUS site for the current amount and enroll at least a week before the flight.
Insurance is a separate, voluntary step. The reason families buy it is the gap between the two health systems. A visitor has no access to Medicare or regular Medicaid, cover from home rarely pays US hospitals directly, and KFF's hospital data puts average US hospital expenses at roughly $3,300 per inpatient day before physician and surgery charges. A short admission adds up quickly.
How long should the plan run for a B1/B2 trip?
Match the policy to the real travel dates, starting on the day of departure from China so the flight and any transit are included. Parents who come to help with a new baby often stay close to the full period CBP grants, and plans change, so think about the longest stay that is realistic.
You do not have to buy the whole period at once. The visitor plans in our comparison can be bought for as little as five days and extended online before they expire, up to about 12 months in total. Extending keeps the same certificate, which matters because a brand new policy would treat anything diagnosed during the first one as pre-existing. Our guide to extending visitor insurance explains the timing. If your relatives might also cross into Canada or Mexico for a few days, choose a plan whose area of coverage includes travel outside the United States.
Which plan type fits parents and older relatives?
There are two families of plan. A fixed benefit plan pays a set dollar amount for each service, such as a daily amount for a hospital room, and you pay the rest. A comprehensive plan pays a percentage of eligible expenses, often 100 percent in network after the deductible, up to the policy maximum. For anyone who could plausibly need a hospital stay, comprehensive is the stronger structure. See fixed versus comprehensive visitor insurance for a worked bill.
Age then narrows the field:
- Under 65: high maximums are available on most comprehensive plans, and acute onset benefits generally run to the policy maximum.
- 65 to 69: Atlas America limits the maximum to $50,000 or $100,000, while IMG's Patriot America Plus certificate keeps its higher options through age 69.
- 70 to 79: most comprehensive plans top out near $100,000. Patriot America Plus has no acute onset cover from age 70. Atlas America keeps it to age 79, and Safe Travels USA Comprehensive limits it to $35,000 in this band.
- 80 and over: maximums drop to $10,000 or $20,000 on the plans we compare, which will not absorb a serious admission. Budget with that in mind.
Is acute onset of a pre-existing condition covered?
Visitor plans exclude pre-existing conditions. High blood pressure, diabetes, heart disease and anything else diagnosed or treated before the policy starts will not be covered for routine care, repeat prescriptions or monitoring. Relatives should bring their full supply of medication, with the generic names written in English.
Some plans add a limited benefit for the acute onset of a pre-existing condition: a sudden, unexpected flare that needs urgent treatment. WorldTrips requires treatment within 24 hours of the onset and the traveler to be under 80. IMG's certificate requires the traveler to be under 70 and the condition to have been stable for at least 30 days before the effective date. A condition that worsens gradually does not qualify under either definition. Treat this as an emergency backstop, and read the wording for the plan you pick.
Can you buy and pay for the plan from the USA?
Yes. This is the simplest route for most families. A son or daughter in the United States completes the application, lists each visitor as the insured person with the passport spelling and date of birth, enters China as the home country, and pays by card. Confirmation and ID cards are sent by email after purchase and can be forwarded to WeChat or saved as a photo.
The plan can start before the flight or after arrival. Buying before departure is the better habit, since nothing that begins before the start date is covered. Plan availability can also vary by the US state entered at purchase, so the quote results on the visitor insurance page are the reliable list for your address.
Is Mandarin language help available at the hospital?
Plan administrators describe their assistance lines as multilingual and available at all hours, but the languages on offer at a given moment are not published. The dependable approach is to prepare. Hospitals that receive federal funding are generally required to provide free interpreter services, so your relative can ask for a Mandarin or Cantonese interpreter at registration, and you can ask to be added to calls with the plan as an authorized contact.
A short bilingual card helps more than anything else. List the diagnoses, medications, allergies, your phone number, and the plan's assistance number. Our guide to seeing a doctor in the US as a tourist explains when to use urgent care rather than an emergency room.

Frequently Asked Questions
Is EVUS the same as travel insurance?
No. EVUS is an online update of biographic information that CBP requires from People's Republic of China passport holders with ten year B1, B2 or B1/B2 visas. It provides no medical cover. Travel medical insurance is bought separately from a private administrator. You need a valid EVUS enrollment to board, and insurance only if you choose to have it.
Can my parents use their Chinese health insurance in the United States?
Generally not in a practical way. Public medical insurance in China is designed for treatment at home, and US hospitals will not bill it. Some private international policies do cover overseas emergencies, so check the wording and the claim method. If payment is by reimbursement only, the family would have to pay the US hospital first.
How far ahead should we buy the policy?
Any time once the flight is booked. A purchase a week or two before departure leaves room to read the certificate and share the ID card. Some plans limit how early you can buy: VisitorSecure cannot be purchased more than three months in advance. Buying earlier does not normally change the premium for the same dates.
What happens if my parents stay longer than planned?
Extend the existing policy online before it expires, as long as the total stays within the plan's maximum period and the stay is still within the date on the I-94. A small extension fee may apply on some plans. Do not let the policy lapse and start again, because a new certificate treats conditions from the earlier period as pre-existing.
