Spain

Spain does not require US citizens and other visa exempt visitors to buy insurance for a short stay, but anyone applying for a Schengen visa must show travel medical cover of at least 30,000 euros. Visitors from outside the EU are billed for treatment, and Medicare usually does not pay abroad, so a travel medical plan with evacuation cover is the practical choice. It is medical cover, not trip cancellation cover.

Do you need travel insurance for Spain?

It depends on your passport. If you need a Schengen visa to visit Spain, insurance is a legal condition of the visa. The EU Visa Code requires cover of at least 30,000 euros for emergency medical care, hospital treatment and repatriation, valid across the whole Schengen area for every day of the trip. If you hold a US, Canadian, British, Australian or other visa exempt passport, Spain does not ask you to buy insurance for a stay of up to 90 days in any 180 days. Your fingerprints and photo are recorded in the EU Entry/Exit System, fully operational since April 10, 2026 according to the European Commission. ETIAS, the planned online travel authorization, had not started as of early October 2026 and is not an insurance rule.

Not being required to hold cover is different from not needing it. The State Department tells US travelers that most care providers abroad do not accept US health plans, and the CDC suggests travel health and evacuation cover for Spain. How good is healthcare in Spain for visitors, and what does it cost?

Spain runs a public system, the Sistema Nacional de Salud, managed by its seventeen regions, alongside a large private hospital sector. Standards in cities and resort areas are high.

  • EU and UK visitors: a European Health Insurance Card, or a UK Global Health Insurance Card, gives access to medically necessary public care on the same terms as residents. It does not cover private clinics or a flight home.
  • Visitors from elsewhere: a public hospital will treat an emergency, then bill you. Private hospitals commonly ask for a card deposit or an insurer guarantee before admission.
  • Medicare: medicare.gov says it usually does not cover care outside the US, and Medicare drug plans do not cover prescriptions bought abroad.

What are the main health and safety risks in Spain?

  • Heat: the CDC warns that heat illness can be deadly and advises limiting activity in the hottest hours. Inland Andalusia, Madrid and Extremadura see extreme summer temperatures.
  • Wildfire smoke and floods: summer wildfires and intense autumn storms occur most years. The October 2024 floods in the Valencia region showed how fast conditions can change on the Mediterranean coast.
  • Insects: the CDC lists dengue and leishmaniasis among diseases present in Spain, so use repellent in warm months.
  • Theft and security: the State Department cites terrorism in its Level 2 advisory and unrest in Ceuta, and pickpocketing is common in Barcelona and Madrid.

What should a travel medical plan include for Spain?

Start from the Schengen minimum even if you do not need a visa, because 30,000 euros is a floor and not a target. A long hospital stay followed by a medical flight to North America or Asia can exceed it.

  • Policy maximum well above the visa minimum, with a deductible you could pay out of pocket. Visa applicants are usually told to pick a zero deductible.
  • Emergency medical evacuation and repatriation, which matters on the Balearic and Canary Islands where the nearest major hospital may be a flight away.
  • Acute onset of preexisting conditions. The international plans on Ombrela, such as Atlas International, Patriot International Lite, Patriot Platinum International and Safe Travels International, do not cover preexisting conditions themselves, and acute onset cover has age limits.   • Adventure sports rider if you plan to ski in the Sierra Nevada or the Pyrenees, dive or climb.

How do you use your insurance if you get sick in Spain?

Call 112 for anything life threatening. For minor problems a pharmacy, marked with a green cross, is the usual first stop, and every town has a duty pharmacy open at night.

For anything that needs a doctor:

  • Phone the assistance number on your insurance ID card before treatment when you can. The insurer can point you to a hospital and may arrange direct payment.
  • Carry your passport and policy number. Hospitals will ask for both.
  • If you pay yourself, keep the itemized invoice, the medical report and proof of payment. You will need them to claim.

Does the 30,000 euro Schengen minimum give enough cover for Spain?

It satisfies the visa rule, but it is a legal floor. A serious accident with surgery, intensive care and a medical flight home can cost more, especially from the Canary or Balearic Islands. Spain is part of the US Visa Waiver Program, so Spanish citizens visiting the United States for tourism or business for up to 90 days normally travel on an approved ESTA instead of a B-2 visa. A longer stay, or a previous refusal, means applying for a B-2 visitor visa at the US Embassy in Madrid.
