Europe (Post-Brexit)

United Kingdom

You do not need insurance to enter the United Kingdom as a visitor, but you will usually pay for hospital care if you are admitted. UK government guidance says overseas visitors are charged 150% of the NHS tariff for chargeable treatment. A travel medical plan with hospital and evacuation cover is the practical way to handle that exposure.

Do you need travel insurance to visit the United Kingdom?

Not by law. The UK government's guidance on the Electronic Travel Authorisation does not ask visitors to show insurance, and border officers do not check for it.

What you do need, if you hold a US passport, is the ETA itself. GOV.UK states that it costs £20 and allows visits of up to 6 months. Carriers have checked for it before boarding since February 25, 2026. British and Irish passport holders are exempt.

The reason to buy cover is cost, not paperwork. The National Health Service is funded for people who live in the UK. If you are a visitor and you are admitted to hospital, you are normally billed. You can compare travel medical plans on Ombrela before you leave.

How does NHS care work for visitors, and what does it cost?

The rules are published on GOV.UK and they are more specific than most travelers expect.

  • Free to everyone: treatment given inside an accident and emergency department, walk in centre, minor injuries unit or urgent care centre, plus GP and nurse consultations in primary care.
  • Chargeable: hospital care once you are admitted as an inpatient or referred to an outpatient clinic. GOV.UK guidance says overseas visitors are charged 150% of the cost of NHS treatment unless an exemption applies.
  • Prescriptions: charges can apply for outpatient treatment.

In practice this means a visit to A&E for a sprained ankle may cost you nothing, while a fracture that needs surgery and a ward bed produces a bill. Treatment that is urgent is not withheld while payment is sorted out, but the debt remains yours.

Private hospitals and private GP services also operate across the UK, mostly in larger cities. They expect payment or an insurer's confirmation before non urgent treatment. To see how inpatient bills can add up, try the medical cost estimator.

What are the main health and safety risks in the UK?

The US State Department reissued its advisory for the United Kingdom on September 9, 2026 at Level 2, exercise increased caution, because of the risk of terrorism. It also notes that petty crime such as phone snatching and pickpocketing is common in tourist areas. Check the current level before you travel, since advisories change.

On the health side, the CDC Travelers' Health page for the UK asks you to be current on routine vaccines, including measles and polio. It lists no yellow fever requirement.

The everyday risks are ordinary ones:

  • Traffic: vehicles drive on the left, so look right first when you step off a kerb. This catches many North American visitors.
  • Weather: the Government of Canada's travel advice for the UK notes severe storms, flooding and, increasingly, heat waves.
  • Hill walking: conditions in Scotland, Wales and the Lake District change quickly. Mountain rescue is volunteer run, and a helicopter transfer still ends at a hospital that bills visitors for admission.
  • Ticks: the CDC lists tickborne illness among the risks for walkers in rural areas.

What should a travel medical plan include for the UK?

The UK has modern hospitals everywhere, so the focus is on paying for care more than on reaching it.

  • Inpatient hospital cover with a policy maximum that comfortably absorbs surgery and several nights on a ward at the 150% visitor rate.
  • Emergency medical evacuation and repatriation, so that a long recovery can continue at home. Read more in our guide to emergency medical evacuation coverage.
  • Acute onset of pre existing conditions if you have a managed condition. Limits and age caps vary by plan; the glossary explains the term.
  • A deductible you can pay from savings, since small A&E visits are often free anyway.

Two details from the plan documents matter for this destination. Some international plans listed on Ombrela cannot be purchased once you are physically in the United Kingdom, so buy before you depart. And at least one plan family limits UK citizens and residents to shorter policy periods. Travel medical insurance is medical and evacuation cover; it is not trip cancellation insurance.

How do you use your insurance if you get sick in the UK?

Match the service to the problem.

  • Life threatening: call 999. Ambulance crews take you to the nearest suitable A&E.
  • Urgent, not life threatening: call 111 or use NHS 111 online. It directs you to an urgent treatment centre, a pharmacist or an out of hours GP.
  • Minor: pharmacists can advise on many common complaints without an appointment.

If you are admitted, the hospital's overseas visitors team will ask about your residency and how you will pay. Give them your insurer's details and call the assistance number on your ID card as soon as you can. The insurer may arrange payment with the hospital, or you may pay and claim later.

Keep every invoice, discharge summary and receipt. Itemised paperwork is what a claim rests on. You can compare plans on Ombrela and check how each one handles payment to hospitals abroad.

Domande frequenti

Is travel insurance mandatory for the UK?

No. The UK government's guidance for the Electronic Travel Authorisation and for visitors does not list insurance as a condition of entry, and it is not checked at the border. It is still worth having, because GOV.UK guidance says overseas visitors are charged 150% of the cost of chargeable NHS hospital treatment, and that bill is yours to settle.

Is the NHS free for American tourists?

Partly. GOV.UK guidance says accident and emergency services and GP consultations are free to everyone. Once you are admitted to hospital as an inpatient, or referred for outpatient treatment, visitors from outside the UK are charged, normally at 150% of the NHS rate. A travel medical plan is designed to pay those eligible costs after your deductible.

Does Medicare cover me in the United Kingdom?

Generally not. Medicare.gov states that Medicare usually does not cover health care while you are traveling outside the US, with only narrow exceptions that do not apply to a trip to Britain. Some Medigap and Medicare Advantage plans include limited foreign emergency benefits, so check yours, then consider a separate travel medical plan for the trip.

Do I need a UK ETA as well as insurance?

They are separate things. The ETA is an entry permission that US citizens and most other visa exempt visitors must hold before boarding; GOV.UK lists the fee as £20 and it does not guarantee entry. Insurance is optional and is never part of the ETA application. Arrange the ETA first, then compare medical cover for your dates.

Visitare gli Stati Uniti partendo da United Kingdom?

If you live in the United Kingdom and plan to visit the United States, British citizens normally travel under the Visa Waiver Program with an approved ESTA for stays of up to 90 days. People who are not eligible apply for a B-2 visitor visa. Neither route requires insurance, but the NHS does not pay for care in the US and a GHIC is not valid there. US hospital bills are far higher than anything billed at home, so visitor medical cover for the full length of your stay is worth arranging before you fly.
