Do you need travel insurance to visit South Africa?

Not to get in. Official travel advice from the US, UK and Canada lists these entry requirements, and insurance is not among them:

  • A passport valid for at least 30 days beyond your departure date, with at least 2 blank pages.
  • No visa for tourist stays of up to 90 days on a US, UK or Canadian passport.
  • A yellow fever certificate only if you arrive from a country with yellow fever transmission. The CDC confirms it is not needed when flying directly from the US.
  • An online traveller declaration. UK government advice says that from July 1, 2026 all travelers entering or leaving South Africa must submit one before travel.

Rules for traveling with children under 18 are stricter than most countries', so check the Department of Home Affairs before you book.

How good is healthcare in South Africa, and what does it cost visitors?

South Africa has two distinct systems.

Private care. The UK Foreign Office describes South Africa as having a very high standard of private medical care. Large private groups, including Netcare, Mediclinic and Life Healthcare, run hospitals in Johannesburg, Pretoria, Cape Town, Durban and other cities. This is where most visitors are treated.

Public care. Public hospitals are under heavy demand. UK guidance warns that standards of treatment and hygiene may differ from what you are used to, and Canadian advice notes that public facilities in rural areas offer limited services.

The catch with private hospitals is payment. The Government of Canada's advice says they require proof of insurance and immediate payment. Without either, admission for anything short of a life threatening emergency can be delayed while a deposit is arranged.

Prices are lower than in the United States but a stay involving surgery or intensive care is still a large bill for an individual, and an air ambulance from a remote lodge costs more than the hospital stay in many cases. The medical cost estimator can help you size a policy maximum.

What are the main health and safety risks in South Africa?

The US State Department's advisory for South Africa is Level 2, exercise increased caution, due to crime, terrorism, unrest and kidnapping. It describes violent crime such as robbery, carjacking and smash and grab attacks on vehicles as common, especially in city centres after dark. Read the current advisory before you travel, as the level and wording are updated periodically.

Health risks named by the CDC:

  • Malaria: present in Kruger National Park, the Mopani and Vhembe districts of Limpopo, the Ehlanzeni district of Mpumalanga and the uMkhanyakude district of KwaZulu-Natal. The CDC recommends prescription prevention medicine for these areas. Cape Town and the Garden Route are outside the malaria zone.
  • Rabies: present in dogs and wildlife. The CDC has also posted a notice about rabies in Cape fur seals, advising you to keep away from seals.
  • Schistosomiasis: avoid swimming in fresh water such as lakes and rivers.
  • African tick bite fever: use repellent on bush walks.
  • Vaccines: hepatitis A is recommended for most travelers, with typhoid for some itineraries.

Other hazards flagged by UK and Canadian advice include frequent fatal road accidents, strong currents at beaches, sudden flooding at low river crossings, and power cuts that affect traffic lights and phone networks.

Do you need extra cover for a safari?

Usually you need the right limits more than a special product. A game drive in a vehicle is ordinary tourism under most travel medical plans. What changes on safari is distance.

Lodges in and around Kruger, in the Kalahari or along the Wild Coast can be several hours by road from a private hospital. Canadian government advice warns that emergency response outside urban centres can be dangerously slow and recommends insurance that covers medical evacuation.

  • Choose a plan with a high emergency medical evacuation limit, and confirm that transport must be arranged through the plan's assistance team.
  • Check the activities list if you plan shark cage diving, bungee jumping, horseback or walking safaris, or scuba diving. Some need an adventure sports rider and some are excluded.
  • Ask your lodge which air ambulance service it uses and whether guests are enrolled.

What should a travel medical plan include for South Africa?

  • Inpatient and intensive care cover at private hospital rates.
  • Emergency medical evacuation and repatriation with a limit sized for an air ambulance and, if needed, a medical flight home.
  • Direct payment or a payment guarantee to the hospital where the plan offers it, because private hospitals ask for money at admission.

Travel medical insurance is medical and evacuation cover, not trip cancellation. Theft of belongings, a common problem for visitors, is generally outside it or covered only by a small baggage benefit.

How do you use your insurance if you get sick in South Africa?

Ask to be taken to a private hospital if your condition allows. Have your insurance ID card and passport ready, and call your plan's assistance line before or during admission so it can confirm cover to the hospital.

If the hospital still wants a deposit, pay by card, keep the receipt and claim it back. Collect itemised invoices and medical reports before discharge; they are harder to obtain once you have flown home.

Does Medicare cover me in South Africa?

No, in almost every case. Medicare.gov states that Medicare usually does not cover health care while you are traveling outside the United States, and the listed exceptions are narrow, tied to emergencies close to the US border or on ships near US ports. If you live in South Africa and plan to visit the United States, South African citizens are not part of the Visa Waiver Program, so you will normally need a B-2 visitor visa and an interview at a US consulate.
