Do you need travel insurance for the Cayman Islands?
There is no legal requirement. The UK Foreign, Commonwealth and Development Office (FCDO) lists a valid passport, and says officials may ask for a return or onward ticket and evidence of funds. Insurance is not part of that list.
The recommendation to carry it comes from several directions. The US State Department advisory for the Cayman Islands, reissued at Level 1 on May 21, 2026, tells travelers to consider medical evacuation insurance. The FCDO tells visitors to hold insurance for local treatment and evacuation. The CDC says the same.
The reason is cost, not scarcity of care. This is one of the better equipped destinations in the Caribbean, and prices reflect that. Medicare states that it usually does not cover care outside the US, so US retirees in particular should not assume their cover travels with them. How good is healthcare in the Cayman Islands, and what does it cost?
The FCDO describes three well equipped hospitals on Grand Cayman: two in George Town and one in the eastern part of the island. The government hospital, run by the Health Services Authority, is the Anthony S. Eden Hospital in George Town, which was formerly called George Town Hospital. Health City Cayman Islands in East End is a private tertiary hospital with its own emergency room.
The Sister Islands are different. Cayman Brac has Faith Hospital for routine medical and dental care, and the hospitals the FCDO lists are all on Grand Cayman or Cayman Brac. A serious problem on Cayman Brac or Little Cayman is likely to mean a transfer to Grand Cayman first.
The FCDO puts the cost question plainly: treatment is expensive, and more serious cases are normally stabilised in the Cayman Islands and then transferred to the USA. Expect to be asked for a card or proof of insurance when you register as a visitor, and contact your insurer quickly if you are referred for treatment, as the FCDO advises.
What are the main health and safety risks for visitors?
- Diving. The Cayman Islands are a major dive destination, and decompression illness is the injury that sets this trip apart. The Health Services Authority says the only recompression chamber in the territory is at its George Town hospital site, staffed on call around the clock. A diver on Cayman Brac or Little Cayman would need to be moved to Grand Cayman for treatment.
- Snorkelling and boat trips. Sandbar excursions and reef snorkelling involve currents, boat traffic and sun exposure. Most incidents are minor, but drowning and near drowning are the events that lead to hospital stays.
- Mosquito borne illness. The CDC lists dengue and Zika, and says there is no malaria. No vaccine is required for entry.
- Roads. Driving is on the left. The FCDO notes a legal alcohol limit lower than in England, with fines and possible imprisonment above it.
- Crime. The FCDO says crime rates are low and advises ordinary precautions. The State Department advisory was at Level 1 when reissued on May 21, 2026. Check the current level before you travel.
When is hurricane season in the Cayman Islands?
The Atlantic hurricane season runs from June 1 to November 30, and the US National Hurricane Center places the peak around September 10. The three islands are low and exposed in the western Caribbean, so a storm can close the airport and suspend non urgent hospital services.
Travel medical insurance pays for treatment and evacuation. It does not refund flights or accommodation cancelled because of a storm, which is what trip cancellation insurance is for. What should a travel medical plan include for the Cayman Islands?
- A policy maximum sized for US prices. Local care is expensive and a transfer to the USA is the normal route for the most serious cases, so choose the limit as if you might be treated in Florida.
- Emergency medical evacuation. This pays for the air ambulance to the US, and for the hop from the Sister Islands to Grand Cayman if it is medically necessary. See how evacuation cover works.
- Scuba wording you have read. Check depth limits, certification requirements and whether hyperbaric treatment is covered. Some plans need an adventure sports add on.
- Direct billing or a clear claims route. Ask whether the insurer can guarantee payment to the hospital. If not, you pay and claim.
- Acute onset of existing conditions, if relevant to you. Age limits and caps differ by plan.

How do you use your insurance if you get sick or injured there?
Call 911 for an ambulance. For a suspected diving injury, say so on the call, because the chamber team has to be brought in.
Once you are stable, phone the assistance line on your insurance ID card. The insurer can confirm cover to the hospital, and it must approve any transfer to the United States before the flight is arranged. Keep the itemised bill, the discharge summary and receipts for any medicines.
For minor problems, a walk in clinic in George Town or along Seven Mile Beach is usually faster than a hospital emergency room. You will normally pay at the desk and claim afterwards.
