Visitor insurance extension works only before your end date: a lapse can reset pre-existing rules. Plan maximums, I-539 timing, and how to renew safely.
To extend visitor insurance, request the extension through your insurer's member portal before the current policy ends. Extending the same policy keeps your original start date, while buying a new policy after a lapse can make any illness that began under the old one a pre-existing condition. Trawick International states plainly that there is no grace period for extensions and that new coverage restarts the pre-existing condition exclusion, the deductible, and the coinsurance. Most plans cap the total length: 364 days for Safe Travels USA plans, 12 months for Atlas America, and up to 24 continuous months for Patriot America Plus. You can compare visitor insurance plans on Ombrela if you need a plan that runs longer.
Key facts at a glance
- Trawick International's Safe Travels USA plans allow extensions only before the termination date, with no grace period, up to 364 days of total coverage.
- On Safe Travels plans, buying new coverage after a lapse restarts the pre-existing condition exclusion, the deductible, and the coinsurance, according to Trawick.
- IMG's Patriot America Plus certificate allows extensions in periods of up to 365 days until the insured reaches a maximum of 24 continuous months.
- WorldTrips' Atlas coverage description sets a maximum duration of 12 months and says any extension is at the insurer's discretion.
- Federal regulation (8 CFR 214.2(b)(1)) admits B-2 visitors for no more than one year and allows extensions in increments of no more than six months each.
- USCIS recommends filing Form I-539 at least 45 days before your authorized stay expires, and Visa Waiver travelers cannot apply to extend.
Can you extend visitor insurance?
Usually, yes. Most visitor medical plans sold for trips to the United States can be extended online while they are still active. Trawick sends an extension notice before the Period of Insurance ends with a link to extend. IMG and WorldTrips handle extensions through their member portals, and in the plan data Ombrela receives from carriers, several plans list a small extension fee of about $5.
The rule that matters is timing. Trawick's plan documents say there are no grace periods for extension, and once coverage has lapsed you may only be able to reapply as a new customer, if you still meet eligibility rules. Treat the end date as a hard deadline and extend at least a week before it, so a declined card or a portal problem does not leave you uninsured.
Should you extend your policy or buy a new one?
Extend if you can. The reason is the pre-existing condition clause. Visitor plans define a pre-existing condition by looking back from the policy's effective date: three years on IMG's Patriot America Plus and Trawick's Safe Travels USA Comprehensive, and two years on WorldTrips' Atlas plans, according to each carrier's plan wording. Anything that existed in that window, diagnosed or not, is excluded.
When you extend, the effective date stays the same. When you buy a new policy, the clock starts again from the new date. Suppose your father develops a heart rhythm problem in month three of his visit and is treated under his first policy. If that policy lapses and you buy a fresh one, the new insurer will treat the heart condition as pre-existing, and ongoing treatment is excluded. At most, an acute onset benefit might pay for a sudden flare up, within its age limits and sublimits.
A new policy also resets the deductible and, on plans with coinsurance, the amount you pay before the plan pays in full. If your parent has already met the deductible, that is money you lose by switching. Read visitor insurance with pre-existing conditions before deciding.
How long can visitor insurance last?
Each plan has its own ceiling. These are the limits stated in the carriers' own documents for plans Ombrela can quote:
- Patriot America Plus (IMG). Each Period of Coverage runs from 5 days to 12 months, and the certificate allows extensions until the insured reaches a maximum of 24 continuous months.
- Atlas America (WorldTrips). The description of coverage sets a maximum duration of 12 months, and WorldTrips says extensions can run up to 364 days from the initial effective date. Any extension is at the insurer's discretion.
- Safe Travels USA Comprehensive, Safe Travels USA, and Safe Travels USA Cost Saver (Trawick). Coverage can be extended provided the total Period of Insurance does not exceed 364 days. Trawick also states that Safe Travels USA is not available to anyone who has been residing in the United States for more than 365 days before the effective date.
Other plans have their own limits, so check the plan page before you buy. If you already know the visit will be long, starting with a plan that allows more time is simpler than switching later. Our Atlas America vs Patriot America comparison covers other differences between two of the most common choices.
Can you get travel insurance for more than 6 months?
Yes. Visitor medical plans are commonly sold for up to 364 or 365 days in one policy, and a six month stay is well within that. Parents visiting on a B-2 visa are often admitted for up to six months, so a policy matching the date on their I-94 record is the usual starting point.
For stays beyond a year, look at plans that allow longer continuous coverage, such as Patriot America Plus. Longer stays also raise the price, because premiums depend mainly on age, length of cover, policy maximum, and deductible. Compare live prices for your age and dates, and read how much visitor insurance costs for what drives the difference.
How do you extend a B-2 stay with Form I-539?
If your visitor wants to stay past the date on their I-94 record, they apply to USCIS on Form I-539, Application to Extend/Change Nonimmigrant Status. USCIS recommends applying at least 45 days before the authorized stay expires. Federal regulation allows B-2 visitors to be granted extensions in increments of no more than six months each, and an extension generally cannot be approved if the stay had already expired when the application was filed, except where USCIS excuses the delay for extraordinary circumstances beyond the applicant's control.
Form I-539 can be filed online or on paper. Fees change, so check the current amount on the USCIS fee schedule (Form G-1055) before filing, and check the USCIS case processing times page, because a decision can arrive after the date you asked to stay until. As of September 2026, USCIS says a court order has it continuing to accept the 08/28/24 edition of Form I-539, so download the form from uscis.gov rather than using an old copy. Travelers who entered on ESTA under the Visa Waiver Program cannot use Form I-539 to extend.
This is general information, not legal advice. Immigration consequences depend on individual facts, so consult an immigration attorney before relying on a pending application to remain in the United States.
How do you keep coverage continuous while the I-539 is pending?
Insurance and immigration run on separate clocks, and both need attention. While the application is pending, keep the visitor insured for every day they remain in the country:
- Extend in step with the stay. If the I-94 date is approaching and the visitor will remain while the I-539 is pending, extend the policy before its end date.
- Keep receipts. Save the USCIS receipt notice and your insurance extension confirmations together.
- Watch the plan ceiling. If the extension would take the policy past its maximum, plan the next policy now so it starts the day after the old one ends.
- If the application is denied, keep coverage until the visitor actually departs, and seek legal advice on the departure timeline.
What happens when you reach the plan's maximum period?
At the ceiling, the policy cannot be extended further, and the only option is new coverage, from the same carrier if its rules allow or from another plan. Buy it so it starts the day after the old one ends, with no gap. Expect the new policy to treat any condition from the previous coverage as pre-existing, and expect a new deductible.
Some plans give a limited cushion after coverage ends. WorldTrips' Atlas description of coverage includes a benefit period of up to 90 days after termination, but only for eligible expenses directly related to an injury or illness diagnosed or treated while the certificate was in effect. Not every plan has one, so check yours before relying on it. For a new plan, compare acute onset terms carefully, because that benefit is what may help if an older condition flares up suddenly.
How to renew visitor insurance, step by step
- Check the end date and ceiling. Look at the ID card or policy documents for the current end date and the maximum total length.
- Log in to the member portal of the administrator named on the card (IMG, WorldTrips, or Trawick), or use the link in the extension email.
- Choose the new end date to match the expected departure, or the requested I-539 date if an application is pending.
- Pay the extension premium, which is charged at the rates in force when you extend, not at your original price.
- Save the confirmation and the updated ID card, and give a copy to the traveler.
- If the plan cannot be extended, buy a new policy that starts the day after the current one ends.
If you need a replacement plan, you can get new visitor insurance quotes and set the start date for the day after the current policy ends. For older parents, visitor insurance for parents over 70 explains the age limits that apply.
Frequently Asked Questions
Can I extend visitor insurance after it expires?
Usually not as an extension. Trawick states there is no grace period, and once a policy lapses you may only be able to buy new coverage if you still meet eligibility rules. New coverage starts a new pre-existing condition lookback, so an illness that began under the old policy may be excluded. Always extend before the current end date, ideally a week or more ahead.
What is the maximum length of a visitor insurance policy?
It depends on the plan. Trawick's Safe Travels USA plans allow up to 364 days in total, WorldTrips sets a 12 month maximum for Atlas coverage, and IMG's Patriot America Plus certificate allows extensions up to 24 continuous months. Check the plan wording, because limits and eligibility rules can change when the carrier updates its documents.
Does extending my visitor insurance reset the pre-existing condition clause?
A genuine extension keeps your original effective date, so conditions that started during your coverage are not treated as pre-existing. Buying a new policy is different: the lookback period starts again from the new effective date. Trawick states that new coverage restarts the pre-existing condition exclusion, the deductible, and the coinsurance, which is why extending on time matters.
Should I buy insurance for the full I-539 period before USCIS decides?
Many families extend in shorter steps that track the real stay, since some policies refund unused time only under set conditions. What matters is that there is no gap on any day the visitor is in the US. This is general information, not legal advice; consult an immigration attorney about the application itself and the departure timeline.
Can ESTA visitors extend their stay and their insurance?
Visa Waiver travelers cannot apply to extend their stay; USCIS lists them among those who may not use the extension process. In an emergency, immigration authorities may grant up to 30 days of satisfactory departure at their discretion. Insurance can usually be extended online, but it cannot change the immigration rules. See do you need travel insurance for ESTA for details.
Long visits deserve continuous cover. Compare visitor insurance plans on Ombrela, choose one whose maximum length fits the whole stay, and set a reminder to extend well before the end date.
