A fixed benefit plan, also called a limited or scheduled benefit plan, sets a dollar cap for each type of care: a daily amount for a hospital room, a set amount for surgery, a set amount for an emergency room visit. Once the deductible is met, the plan pays the scheduled amount and you pay whatever the provider charges above it. Because the insurer's exposure is capped service by service, the premium can be lower.
The catch is that US prices are not scheduled. If a hospital day costs more than the plan's daily limit, the difference is yours, even if you chose a large overall maximum. None of these trims makes a plan bad. A healthy traveler in their 30s on a two week trip who will use in network hospitals may be comfortable paying 20% of the first $5,000. A parent in their 70s with high blood pressure should read the acute onset rules before looking at price at all.
How much policy maximum do you need in the US?
The policy maximum is the most the plan will pay. There is no legal minimum for visitors, so the useful question is what a serious event would cost. Using KFF's estimate of about $3,297 in hospital expenses per adjusted inpatient day in 2024, a single week in hospital represents more than $23,000 in hospital expenses before surgeon, anesthesia or ambulance fees, and KFF's state figures vary widely around that national average.
Lowering the maximum does reduce the premium, but weigh the dollars you save against the uncovered exposure you add. Run a few scenarios in the medical cost estimator and compare the result with the maximums on your quotes.
Age narrows the choice. WorldTrips lists Atlas America maximums of up to $2,000,000 at 64 and under, but $50,000 or $100,000 at 65 to 79. The Visitors Care listing notes a $10,000 coverage limit for travelers over 80. For an older visitor, the realistic comparison is often between a handful of plans rather than the whole market.
Does a higher deductible make visitor insurance cheaper?
Usually, yes. Raising the deductible lowers the premium because you agree to pay the first part of each claim yourself. The trade off works well when you could comfortably pay that amount in cash, and badly when a deductible you cannot afford stops you from seeking care early.
